What sets this dashboard suite apart is its ability to integrate data from multiple sources. This includes WellSky CarePort solutions, electronic health records (EHR), and vital claims data. With all this information at their fingertips, acute care providers are in a better position to make informed decisions quickly.
One of the standout features of the dashboards is their intuitive visualizations. These allow healthcare professionals to easily comprehend referral metrics and trends, empowering them to take action where it’s most needed. The dashboards provide real-time insights into critical metrics such as the average length of stay in hospitals and the response times for referrals, which can drastically influence patient care.
WellSky has collaborated with The Center for Case Management (CFCM) to offer the WellSky Clinical Performance dashboard. Skilled nursing facility (SNF) referrals are up 3%, while home health referrals have surged by 6%.
Despite this growth in referrals, acceptance rates remain lower than pre-pandemic levels. This makes it challenging for acute providers to secure timely post-acute care, further complicating the situation. The average hospital length of stay before discharge has also seen a rise, continuing to impact hospital expenses and patient satisfaction.
The rise in hospital stays also tells another story, particularly for those discharged to institutional care like SNFs and home health services. Since 2019, the average length of stay (ALOS) has increased by 12% for discharges to SNFs, and by 7% for discharges to home health. Staffing shortages in nursing homes are worsening the situation, with over half of these facilities denying patients due to these labor challenges.
As we look ahead, the demographic shift expected by 2030—with an estimated 73 million baby boomers set to be aged 65 and older—will only elevate the importance of effective care coordination. In response, WellSky aims to leverage advanced technology and artificial intelligence to streamline care coordination and improve patient outcomes amidst rising healthcare complexities.
