As homelessness has surged by over 100% recently, local agencies are collaborating to offer emergency assistance and individualized care to those affected. With ongoing commitment, this initiative aims to create a brighter future for families struggling with housing instability.
In a heartwarming move to help families in need, Chautauqua County has rolled out the Family-Centered Case Management (FCCM) Initiative. This fresh program is all about addressing the rising tide of homelessness that has hit the region hard lately. Funded by the New York State Office of Temporary and Disability Assistance, this initiative is a first for the county, bringing together the Department of Mental Hygiene and the Department of Social Services in a unique and comprehensive manner.
The main objective of the FCCM Initiative is to provide wraparound support tailored specifically for families experiencing homelessness. Currently, about 50 families are being provided with temporary housing, but that number is constantly changing as new families find themselves in need of help. Each day, caseworkers alert the FCCM team when a family enters temporary housing, ensuring they get hooked up with the support services they need right away.
This initiative revolves around the idea of creating individualized case management plans. This means families get the help they specifically require to tackle their unique challenges head-on. From mental health to job training, the program aims to address various aspects of family life and offers ongoing support that includes regular follow-ups to monitor progress and engage families as needed.
